Care Management and Discharge Planning
We coordinate your plan of care throughout your hospitalization and help you and your family prepare for your discharge and follow up care.

Clinical Standards and Guidelines
We develop guidelines for care based on research-based, professional standards of practice. We also work with members of the patient care team to assure that the care provided at Sutter Solano Medical Center is consistent with those standards.

Social Services
We provide consultation on your psychological and social concerns and provide referrals to available community resources.

Education
We coordinate patient and family education during your stay, provide learning resources, and offer community education programs.
